Gwen’s Quality Care is a home care agency committed to enhancing the quality of life for our clients. Founded in 2019, by owner Gwendolynn Newton who has over 30 years of experience in this field. Gwen’s Quality Care employs a small staff of trained Homemakers, Nursing Assistants, and Registered Nurses who are readily available to provide home care services for you or a loved one. Services are available 24 hours a day, 7 days a week - holidays and weekends included! We will customize a care plan to meet your needs.

Our History:
Gwen’s Quality Care has been providing compassionate, dependable care as an alternative to nursing home placement for over 30 years. Our company name describes our mission perfectly. Since 1989, we have provided the means for hundreds of people to remain in their own homes instead of relocating to a health care facility. Our committed and well-trained staff makes it possible to keep people comfortably and safely in their own homes and communities. 


Our Staff: 

Our company is fully insured and bonded; all of our staff are thoroughly trained, tested, and background-checked before ever entering a client’s home. Field staff are required to attend in-service training on a regular basis and are always closely monitored by our staffing supervisors. Our comprehensive training program exceeds State and Federal minimum standards and requirements. In fact, our training program has even been used as a model for other home care agencies! Our intense commitment to quality has allowed us to partner with local and federal funded home care programs. 

 

We operate one branch location, providing services throughout the state of Tennessee. Our client list is constantly expanding. Our small size does not prevent us from having a voice in the industry, which benefits and protects our clients' rights to quality and services. It also allows us to manage costs effectively and ensure that there is always coverage for cases. Even though our operation is overall quite small, our office is managed at the local level by a trusted manager. The branch manager is in constant contact with the staff and patients in order to facilitate the strict enforcement of our standards. Through this model of leadership and management, we are able to provide the type of personal service that will make us a leader in our industry!

Employees applying for positions that require licenses and/or certifications must ensure these are currently in the state for which they are applying for employment. 

Homemaker, Companion, Personal Care Attendant/Assistant, Home Care Aide

These positions provide non-medical assistance with the activities of daily living--such as personal care, housekeeping, laundry, meal preparation, and errands, to name a few. 

Some of the qualifications for these positions include: 

  • Basic knowledge of home management skills. 

  • High school diploma OR GED OR at least one (1) year of documented supervised service in an in-home supportive services program or in a comparable human services program. 

Certified Nursing Assistant (CNA), Home Health Aides in Tennessee

In addition to providing homemaker, personal care and respite duties, the main focus of this position is to encourage and provide complete client satisfaction via quality non-medical care to clients in their homes, and to communicate client information to administrative staff on a consistent basis. 

Some of the qualification for CNAs include: 

  • Certification in the state of employment. 

  • High school diploma OR GED OR at least one (1) year of documented supervised service in an in-home supportive services program or in a comparable human services program. 

Licensed Practical Nurse (LPN)

Working under the direct supervision of the Registered Nurse and Branch Manager, LPNs provide nursing services to clients as dictated by each client’s plan of care. 

Some of the minimum qualifications for LPNs include: 

  • Must be a Licensed Practical Nurse in the state of employment, on active status and in good standing.

  • Must have at least one year of experience in the health care industry. 


Registered Nurse (RN):

Registered Nurses ensure that quality care is provided to clients while maintaining clinical compliance. Registered Nurses may also oversee the training of field staff and supervise the work of LPNs. 

Some of the minimum qualifications for RNs are: 

  • Must have a current RN license. 

Supervisor:

While maintaining our high standard of quality services, supervisors coordinate client care from referral sources through clients and employees. Harmonization occurs through constant supervision of employees, assessments with clients, and ongoing communication with all parties. Supervisors ensure that worker schedules are being adhered to and that the proper electronic visit verification procedures are being followed. 

Some of the minimum qualifications for Supervisors are: 

  • Must have supervisory experience

  • Must have previous experience working with the elderly and/or disabled, or must display a compassionate attitude for that constituent.

Whether you are coming home after a stay in the hospital or simply need some assistance with the activities of daily living, Gwen’s Quality Care can help! We can customize a care plan that fits into your schedule and meets your needs - whether it is a couple of hours a week or around-the-clock care. 

Homemakers, Home Care Aides, Companions:

The primary function of our Homemakers, Home Care Aides, and Companions are to provide personal care and assistance with the activities of daily living (ADL), such as: 

  • Light housekeeping

  • Laundry

  • Bathing, Dressing, and Grooming

  • Meal Preparation, including shopping and assistance with meal planning

  • Errands or Transportation

  • Companionship - available 24 hours a day, if needed

Skilled Services

Home Health Aides:

The primary function of the Home Health Aide is to provide personal care and assistance with activities of daily living: 

  • Bathing and dressing

  • Hair and nail care

  • Oral hygiene

  • Skincare

  • Assistance with ambulation and transfers

  • Medication reminders

  • Maintaining a clean, safe environment

Home Health Aides work in your home under the supervision of the Registered Nurse. A Registered Nurse will come to your home to perform an initial evaluation and together with you and your physician; an individualized Aide Care Plan is developed to assist your personal needs. The Registered Nurse will make ongoing visits to your home to evaluate how well the Plan of Care is meeting your needs and to supervise the care provided by the Home Health Aide. 

Our Home Health Aides go through extensive training programs and complete a Competency Evaluation program before working in your home. Additionally, they attend quarterly in-service programs, designed to keep them up-to-date on procedures and skills. 

Nurse Assistants (CNAs)

Private Duty Certified Nursing Assistants (CNA's) go through intensive training and are eligible to work following their graduation. They also attend our quarterly training programs conducted by our staff Registered Nurses. In some states, intensively trained CNA's are hired under Homemaker Services, so be sure to ask if you are looking for this type of service. 

In addition to providing help with the activities of daily living, CNA's also:

  • Assist with Medication Reminders

  • Oversee Dietary Guidelines with Meal Preparation

  • Personal Care - Bathing, Dressing, and Grooming 

  • Light housekeeping

  • Laundry

  • Meal Preparation, including shopping and assistance with meal planning

  • Errands or Transportation

  • Companionship - available 24 hours a day, if needed

Skilled Nursing Care (RNs & LPNs) 

Gwen’s Quality Care can organize a complete Plan of Care for you. When you or a family member are affected by advanced age, have a temporary or long-term physical or mental disability, or are simply recovering from surgery or an illness, Gwen’s Quality Care can design a program to meet your specific needs. Depending on your situation, Gwen’s Quality Care provides services to enhance the quality of life. 

Skilled nurses include both Registered Nurses and Licensed Practical Nurses who work under the supervision of the Registered Nurse. As a Team, they provide the treatments and teaching that will assist you in managing your healthcare needs.

Gwen’s Quality Care is committed in providing quality home care services. As a future Medicaid Certified Agency, our nurses will be able to write a Prior Authorization to customize a plan of care for your health care needs. We will offer a full range of in-home services such as Skilled Nursing, Home Health Aide, Attendant Care, and Homemaker. Our goal will be to make daily living easier and more comfortable for people recovering from hospitalization, surgery and those with special medical, physical or behavioral needs as well as those with chronic illnesses. 

Care for Adults and Children with Developmental Disabilities

People who have Developmental Disabilities are offered the option of programs that range in intensity and set according to their individual goals and desires.

 

Residential Services

Gwen’s Quality Care assists individuals with securing housing within the community. Gwen’s Quality Care supports the individual in acquiring skills and participating in community life according to their own interests and abilities. Because the model is focused on the goals of one-person, residential services allow the individual to make choices and selectively participate in community activities. 

Family Caregiver Support

Gwen’s Quality Care offers Family Caregiver Support Respite for individuals who live with their families, allowing both parties a healthy change in routine. 

Children and Families

Gwen’s Quality Care provides the protection and welfare of at-risk children in numerous ways, depending on the situation. Services range from transitional programs to residential programs. 

Traumatic Brain Injury (TBI)

For people who have sustained brain injuries, Gwen’s Quality Care provides a wide range of services and support that adapt to their changing needs. These services are designed to assist these individuals in living full and productive lives, as independently as possible. 

Gwen’s Quality Care is committed in providing quality home care services. As a future Medicaid Certified Agency, our nurses will be able to write a Prior Authorization to customize a plan of care for your health care needs. We will offer a full range of in-home services such as Skilled Nursing, Home Health Aide, Attendant Care, and Homemaker. Our goal will be to make daily living easier and more comfortable for people recovering from hospitalization, surgery and those with special medical, physical or behavioral needs and those with chronic illnesses. 

DCFS Services

Transportation

If the care plan for you or a loved one includes assistance with transportation, the staff of Gwen’s Quality Care is happy to assist you. Commonly, this is to aid our participants in keeping medical appointments or to run errands such as food shopping. We can even help our clients navigate public transportation where appropriate. The availability of transportation assistance varies by location and care plan. We do ensure that any staff that must drive a participant are currently licensed and insured to do so. 

Visitation

These services are designed to enhance the visitation process through weekly planned visits between parent and child, thereby maintaining the parent/child bond and promoting a timely plan for the child to return to the home of the parent whenever the goal is to return home. 

Habilitations

These Services are designed to assist parents or other caregivers to maintain or reunify their family unit. These services are provided primarily in the client’s home and are protective services to assist in maintaining the family and strengthening the ability of the parents or caregivers to provide adequate childcare and to improve parenting skills. Additionally, family habilitation services may be provided to assist caregivers during times of family crisis and are used to support reunification efforts. 

Drug Testing

We plan to establish a DCFS contract, to provide Toxicology Laboratory testing for individuals involved with DCFS due to an investigation or indicated case of child abuse and/or neglect. Individuals will be referred for testing by DCFS.
